Anybody living within our catchment area is welcome to register at Summercroft Surgery; we are unable to accept patients from outside of the catchment area.
How to Register
To register with our practice please follow the link below to download our registration form; alternatively you can complete our Online Registration Form.
After you have completed the form you must attend the surgery within 7 days with one form of identification and proof of address alongside the registration form.
Acceptable identification includes
– Passport
– Birth Certificate
– HC2 Certificate
– Rough sleepers’ identity badge
– Hostel Registration/mail forwarding letter
-Rough sleepers’ identity badge
-Hostel Registration/mail forwarding letter.
Until we have seen your identification you will not be registered at the practice.

Practice Boundary
We are happy to register new NHS patients within our practice boundary area.
Before attempting to register with the practice, please ensure you live within our service boundary by entering your postcode below.
Catchment Area
Temporary Residents
If you are not registered with the practice but need treatment from a surgery, you can request to register as a temporary patient.
Please collect a Temporary Patient Form from the surgery reception.

Overseas Visitors
Those who visit the UK from overseas must pay for their care when they are in England.
In 2015 changes were made to how the NHS charges overseas visitors for healthcare so the NHS does not lose out on income for the services these patients have received.
